What can you put in your Official Military Personnel File OMPF?
Keeping your military records up to date is crucial especially when trying to get promoted to SFC and above. Not all
documents are authorized for filing in your Official Military Personnel File OMPF.  Be sure to review OMPF Document
Filing Reference before sending documents for file. All unauthorized documents will be destroyed.

WHEN SUBMITTING DOCUMENTS FOR PLACEMENT ON MY OMPF, CAN I COMBINE TWO DOCUMENTS ON ONE PAGE?
Do not put more than one document on a page, except for Correspondence Course Completion Notices. We need both Part A
and Part B of the Notices to be able to file these on the OMPF.

WHAT IS THE CORRECT WAY TO SUBMIT COLLEGE TRANSCRIPTS FOR POSTING TO MY OMPF?
You MUST send an Official Transcript with a School Seal or the Registrar’s Signature. We will return these originals to you. If you
want these documents returned to your home address make sure your send your return address along with your transcripts. If
you have the school mail your transcripts directly to SRDC, ensure they also provide us with your return address. College
transcripts MUST be legible.

HOW CAN I MAKE CORRECTIONS TO MY OMPF?
Click here, fill out the request form, on the form please provide us with a detailed description of your request. Hit submit and it
will come to our Personnel Action Branch (PAB).

HOW CAN I ADD DOCUMENTS TO MY OMPF?
To add new or missing documents take them to your S1 or PSB. Check to see if they will transmit them to SRDC via Digital
Sender or Fax to File. If so the document should be on your OMPF within 72 hours. Check your OMPF to make sure we received it.

I WAS AWARDED A GERMAN MARKSMANSHIP AWARD IN SILVER AND A GERMAN PARACHUTE BADGE. CAN THESE BE
ENTERED ON MY OMPF?
These awards can be entered on your OMPF if approved by a Commander in the rank of BG or higher or Colonel level
Commanders who exercise General Court Martial authority. You must submit a letter or
DA 4187, through channels, for approval
to wear. (See AR 600-8-22, Chapter 9 Section V, Paragraph 9-29 Foreign Decorations, Paragraph 9-30 Foreign Badges,
Appendix D Table D-1 and AR 600-8-104, Table 2-1 (Awards).

SHOULD I SEND THE ORIGINAL DOCUMENTS OR CAN I SEND A COPY TO BE FILED IN MY OMPF?
You should send a legible copy of all documents except your college transcripts. We do need an original copy of your college
transcripts.

HOW CAN I BE SURE YOU RECEIVED MY DOCUMENTS?
Send them by registered mail. We will not e-mail, fax or mail a message to you stating we received your documents.

DO PROMOTION 4187s NEED TO BE SENT TO SRDC FOR FILING?
Promotion to E2, E3 and E4 can be submitted for filing upon promotion to E5.
When submitting these documents please send a copy of your E5 orders or a short  
statement that you have been promoted to E5.
